"ATHENA is truly a landmark trial, that marks a paradigm change for the management of atrial fibrillation," said Prof Christopher Cannon, a Senior Investigator in the TIMI Study Group at Brigham and Women's Hospital, Harvard Medical School, who was not involved in the study.

"Atrial fibrillation is a very common disease, and our prior treatment options have been focused only on symptom relief and a hope to not do harm, which has been the problem with prior antiarrhythmic drugs. Now, with a highly significant reduction in cardiovascular hospitalisation or death, as well as a 45 per cent reduction in arrhythmic death or 30 per cent cardiovascular death, dronedarone may become a first line treatment of atrial fibrillation."

The ATHENA study results were presented by Prof Stefan Hohnloser last week (15th May 08) at the Heart Rhythm Society's 29th Annual Scientific Sessions in San Francisco, USA. The study results show that dronedarone significantly decreased the risk of cardiovascular hospitalisations or death from any cause by 24 per cent (p=0.00000002), meeting the study's primary endpoint.

Results of the primary endpoint were consistent across all the pre-specified clinically relevant subgroups.

The most frequently reported adverse events of Multaq(R) vs. placebo in the ATHENA trial were gastro-intestinal effects (26 per cent vs. 22 per cent), skin disorders (10 per cent vs. 8 per cent, mainly rash) and increased blood creatinine (4.7 per cent vs. 1 per cent). The mechanism of blood creatinine increase (inhibition of creatinine secretion at the renal tubular level) is well defined. Compared to placebo, Multaq(R) showed a low risk of pro-arrhythmia and no excess of hospitalisations for congestive heart failure. There was a similar rate of study drug discontinuation between the 2 study groups.

Atrial fibrillation is a major cause of hospitalisation and mortality and affects about 2.5 million people in the United States, as well as 4.5 million people in the European Union and is emerging as a growing public health concern due to an aging population. Patients suffering from atrial fibrillation have twice the risk of death, an increased risk of stroke and cardiovascular complications, including congestive heart failure.

ATHENA, was the largest double blind randomised study in patients with atrial fibrillation, and conducted in more than 550 sites in 37 countries and enrolled a total of 4,628 patients. The landmark ATHENA trial is the first morbidity-mortality study as part of the Multaq(R) phase III clinical development program, which also included five other multinational clinical studies: an initial study, ANDROMEDA, conducted in patients with severe congestive heart failure and a recent decompensation, and a total of 4 international studies in atrial fibrillation: EURIDIS/ADONIS, ERATO, and the ongoing DIONYSOS trial.

About Atrial Fibrillation / Flutter

Atrial fibrillation is a major cause of hospitalisation and mortality and affects about 2.5 million people in the USA and 4.5 million people in the European Union. The most common symptoms of atrial fibrillation include palpitations (a rapid, irregular, "flopping" movement or pounding sensation in the chest or neck), shortness of breath, dizziness and feeling of heaviness, or constriction in the chest. The disorder may even be more common than diagnosed, as patients may experience atrial fibrillation episodes that either do not cause symptoms or are not documented during their visits to the doctor.

The landmark ATHENA study is a randomised, placebo controlled, international multi-center study that evaluated for the first time a treatment on top of standard background therapy for the management of patients with atrial fibrillation in reducing morbidity and mortality by preventing cardiovascular hospitalisations or death from any cause. The study included 4,628 patients, which make it the largest ever outcome study of an anti-arrhythmic treatment for atrial fibrillation.

The ATHENA study objectives were to show a potential benefit of Multaq(R) on the primary composite endpoint of all-cause mortality combined with cardiovascular hospitalisation as compared to placebo. The pre-specified secondary endpoints were death from any cause, cardiovascular death and hospitalisation for cardiovascular reasons.

Furthermore from the study secondary endpoints, Multaq(R), showed a significant decrease in the risk of cardiovascular death by 30 per cent (p=0.03) on top of standard therapy, including rate control and antithrombotic drugs, in patients with atrial fibrillation or atrial flutter. Multaq(R) also significantly decreased the risk of arrhythmic death by 45 per cent (p=0.01) and there were numerically fewer deaths (16 per cent) from any cause in the dronedarone group compared to placebo (p=0.17).

First cardiovascular hospitalisation was reduced by 25 per cent (p=0.000000009) in the dronedarone group. The pre-specified safety endpoint was the incidence of treatment emergent adverse events (time of observation for treatment emergent adverse events) including: all adverse events, serious adverse events, adverse events leading to study drug discontinuation.

The atrial fibrillation or atrial flutter patients studied were either 75 years of age or over (with or without cardiovascular risk factor) or were below 75 years of age with at least one additional cardiovascular risk factor (hypertension, diabetes, previous cerebrovascular event, left atrium size greater than 50 mm or left ventricular ejection fraction lower than 40 per cent). Patients suffering from decompensated heart failure were excluded from the study.
Patients were randomised to receive Multaq(R) 400 mg BID or placebo, with a maximum follow-up of 30 months.

Sanofi-aventis (EURONEXT: SAN and NYSE: SNY) announced today that findings from the landmark ATHENA study showed that Multaq(R) (dronedarone), a potential therapy for the treatment of patients with atrial fibrillation or atrial flutter, decreased the risk of cardiovascular hospitalisations or death from any cause by a statistically significant 24% (p=0.00000002), meeting the study's primary endpoint. The ATHENA results will be presented at the late breaking clinical trial session of Heart Rhythm 2008, the Heart Rhythm Society's 29th Annual Scientific Sessions in San Francisco, USA.

For the first time in twenty years of clinical drug trials in atrial fibrillation, an investigational medicine, Multaq(R), showed a significant decrease in the risk of cardiovascular death by 30% (p=0.03) on top of standard therapy, including rate control and antithrombotic drugs, in patients with atrial fibrillation or atrial flutter. Multaq(R) also significantly decreased the risk of arrhythmic death by 45% (p=0.01) and there were numerically fewer deaths (16%) from any cause in the dronedarone group compared to placebo (p=0.17). New 300mg Loading Dose Tablet for Plavix(R) Receives Positive Opinion From the European Committee

- A new Step Forward may Help Increase Appropriate Early use of Plavix(R) by Simplifying Administration in Acute Coronary Syndrome Patients

Princeton, N.J. and Paris (ANTARA/PRNewswire-AsiaNet) - Sanofi-aventis (EURONEXT: SAN and NYSE: SNY) and Bristol-Myers Squibb Company (NYSE: BMY) announced today that the Committee for Medicinal Products for Human Use (CHMP) of the European Medicines Evaluation Agency (EMEA) has issued a positive opinion recommending approval of the 300mg tablet of the antiplatelet Plavix(R) (clopidogrel bisulfate). This positive opinion from the CHMP needs to be ratified by the European Commission in the coming months before final approval.

The new 300mg tablet will facilitate the use of the approved loading dose of Plavix(R) and the early initiation as recommended by national and international guidelines in appropriate patients with acute coronary syndrome (ACS), including those with unstable angina / non-ST segment elevation myocardial infarction (managed with percutaneous intervention (PCI) with or without stent or medically managed) and ST segment elevation myocardial infarction patients. The 300mg tablet is bioequivalent to four 75mg tablets of Plavix(R).

Sanofi-aventis and Bristol-Myers Squibb are committed to further studying the optimization of the Plavix(R) loading dose in ACS patients managed with an early invasive strategy in the ongoing large international clinical trial known as 'CURRENT'. CURRENT is the only randomized controlled trial designed to investigate in 14,000 patients, whether the use of a clopidogrel 600mg loading dose, followed by an intensified clopidogrel dosing regimen (6 days at 150mg follow by 75mg) yields improved clinical outcomes as compared to clopidogrel 300mg loading dose followed by 75mg in ACS patients managed with an early invasive strategy. The primary endpoint of CURRENT is the reduction of a composite endpoint of cardiovascular death, stroke and myocardial infarct (MI) and the results are expected by the end of 2008.

Plavix(R) is an antiplatelet agent which prevents platelets from sticking together and forming clots in the arteries and is the only widely available prescription antiplatelet approved to reduce the risk of heart attack and stroke across the entire spectrum of ACS.

Plavix(R) is indicated for the reduction of atherothrombotic events in patients with a history of recent MI, recent stroke or established peripheral arterial disease (P.A.D.) Plavix(R) is also indicated for patients with NSTEMI ACS, including those who are to be managed medically and those managed with PCI (with or without a stent) or CABG. Additionally Plavix(R) is indicated in STEMI patients.

For ACS patients, European Guidelines (ESC Guidelines for NSTEMI ACS 2007) recommend pre-treatment of unselected patients with Plavix(R) before angiography results for better outcome of PCI.

The early and long term efficacy and safety profile of Plavix(R) is well established in clinical trials involving more than 100,000 patients across the full spectrum of ACS, recent MI, recent ischemic stroke and established P.A.D, with real-world experience since its launch in more than 70 million patients worldwide.

Clexane(R)/Lovenox(R) Approved in Japan

Paris (ANTARA/PRNewswire-AsiaNet) - Sanofi-aventis announced today that the anticoagulant Clexane(R) (enoxaparin sodium injection) has been approved for marketing in Japan by the Ministry of Health, Labour and Welfare for the prevention of venous thromboembolism (VTE) in patients undergoing orthopaedic surgery of the lower limbs such as total hip replacement, total knee replacement and hip fracture surgery.

Venous thromboembolism is a frequent and preventable complication among patients hospitalized for orthopaedic surgery. Deep vein thrombosis (DVT) and pulmonary embolism are common manifestations of VTE and can significantly impact morbidity and mortality in surgical patients. The Japanese VTE guidelines state that, without prophylaxis, between 27% and 50% of orthopaedic surgery patients may suffer from deep vein thrombosis. Among the hospitalised patients at risk for VTE, 64% are those undergoing surgery(1).

Outside Japan with over 200 million patients treated in more than 100 countries, Clexane(R) / Lovenox(R) (enoxaparin sodium) is the most extensively studied and most widely used low-molecular-weight heparin in the world. In venous thrombosis, Clexane(R) / Lovenox(R) is recommended by international guidelines not only in orthopaedic and general surgical patients at high and moderate risk of VTE, but also for acutely ill medical patients, and is an important treatment option for millions of patients at risk of VTE(2). In arterial thrombosis, Clexane(R) / Lovenox(R) has demonstrated its effectiveness in preventing, in conjunction with other treatments, the ischaemic complications of unstable angina and myocardial infarction and is also recommended by international guidelines(2).

About venous thromboembolism (VTE)
Venous thromboembolism is a general term used to describe the formation of a blood clot (thrombus) that blocks a vein. This may occur in any part of the venous system, but the most common manifestations are deep-vein thrombosis (DVT), usually in the leg, and pulmonary embolism (PE). PE is a potentially life-threatening complication of DVT; anyone who experiences DVT is at risk of a PE, which occurs when part or all of a blood clot in a deep vein breaks away from where it originally formed and travels through the venous circulation, eventually becoming lodged in the lungs. This blocks the flow of blood in the lungs and is often fatal.

The total annual burden of non-fatal symptomatic VTE in the European Union, which included DVT and PE, is estimated to exceed 1.5 million events, including more than 500,000 deaths(3). This represents more than the double of the combined deaths due to AIDS, breast and prostate cancer and transport accidents(4).

In the United States, up to 2 million DVT events occur each year, and the development of PE causes up to 200,000 deaths annually(5). Fatal PE is the leading cause of sudden death among hospitalized patients and contributes to up to 10% of in-hospital deaths(6).

In Japan, the population survey report by the Ministry of Health, Labour and Welfare showed that annual number of deaths from PE had sharply increased between 1988 and 2001 (591 to 1749 deaths).
